I'd say the top three problems are over-engineering, neglecting performance considerations and inflexibility. Over-engineering seems endemic in J2EE. I think it's because there's so much infrastructure on offer. Developers tend to think that because complex technologies like EJB exist, they ought to use them whether it's really necessary or not. Good engineering practice doesn't mean using the most complex approach.
